Referrals

Dr. Pung is a specialist physician.  It is the patient's responsibilty to obtain and provide a referral.  If you are unsure if your plan requires a referral, please call the number on the back of your card.

It is the patient's responsibilty to contact their insurance regarding their benefits, i.e. deductible, copays, coverage of benefits, HSA expenses, etc.
