THIS IS A SCENT-FREE OFFICE!

Due to many patients with scent sensitivities, please refrain from using strong, odor-emitting items such as: perfumes, colognes and scented lotions.

 

NO FOOD/DRINKS IN THE OFFICE

Due to patients with severe food allergiest, please do not bring food/drinks into the office.

 

RESCHEDUALING APPOINTMENTS

All patients must provide at least 24 hours notice when rescheduling appointments to avoid being charged a fee.

 

CANCELLING APPOINTMENTS

All patients must provide at least 24 hours notice when cancelling appointments or you will be charged a fee.

 

REQUESTS FOR SCHOOL FORMS & MEDICAL RECORDS

Please allow 3-5 business days for our office to process your request for school/camp forms and medical records (lab, blood test and x-ray reports, allergy testing results, etc.).

The fee for forms is $10 each form.

 

If the forms are needed earlier, you can pay a charge of $20 each for expedited forms. The fee for non-expedited forms will remain $10 and be completed in 3-5 business days.

 

Options for retrival of school forms:

- Facsimile: Provide a fax number for us to fax the forms to a parent, school, or camp.
- Postal Mail: Provide a stamped, self-addressed envelope for us to mail the forms.
- Pick-Up: Physically pick-up the forms from the office.
